Diverse Community Care

Diverse Community Care provides choice and peace of mind for all people needing support to stay in their own home and chosen community. Our friendly and professional team believes in a collaborative approach when working with older people, people with a disability, and their carers, so that we can create tailored care and support plans based on the needs and preferences of each person. Diverse Community Care assists people to access services and networks within their community and creates understanding about the range of options available. Individual circumstances and sensitivities are respected and our team has a genuine passion for providing positive lifestyle outcomes which enable people to remain living in their home. A not-for-profit organisation, Diverse Community Care was established in July 2010 following the merger of Midwest Community Care and Community Independence Support Services. Services are available in the Local Government Areas of Auburn, Bankstown, Blacktown, Holroyd, Hunters Hill, Parramatta, Ryde and The Hills.
